To obtain a copy of a Medical Record from LASUTH Healthcare System facility, you must complete and submit an Authorization for Release of Confidential Medical Records Form.

Our Physician Referral Service is available 24hours a day, 7days a week.

Patients need to visit the hospital in person in order to schedule an appointment for a full medical checkup.
